Dieses Template gehört zur Infobox Ballot und dient zur Angabe eines einzelnen Ballots (Abstimmungsdokuments).
Der Aufruf wird zwischen Infobox Ballot Begin und Infobox Ballot End eingebettet und ist wie folgt:

mit folgenden Parametern:

| Realm | Realm oder Realms, für die die Abstimmung gültig ist. Hier sind folgende Parameter zugelassen:

| Date | Datum der Abstimmung im Format dd.mm.jjjj |
| Status | Status der Abstimmung, hier sind zugelassen
Je nach Statusangabe ändert sich das entsprechende Logo |
| Otherdocuments | URLs zu anderen Dokumenten, z. B. dem Download des PDFs |
