Dieses Template gehört zur Infobox Contributors und dient zur Angabe eines Kontributoren (Beitragenden).
Der Aufruf wird zwischen Infobox Contributors Begin und Infobox Contributors End eingebettet und ist wie folgt:

mit folgenden Parametern:

| Logo | Logo des Beitragenden als Wiki Datei, z. B. Logo = Logo-hl7.jpg |
| Name | Name des Beitragenden (Organisation oder Person) |
| Location | Sitz des Beitragenden |
