TXA – Dokumentinformationen

Im TXA-Segment werden die wichtigsten Meta-Daten zu einem Dokument untergebracht.

In the TXA segment the main meta-data is stored to a document.

TXA-1 Segmentnummer

Dieses Feld enthält eine fortlaufende Nummer zur Identifikation mehrerer TXA-Segmente innerhalb einer Nachricht. Es wird nicht zur Übermittlung der Set-ID für Dokumentgruppen verwendet.

This field contains a serial number for the identification of several TXA-segments within a message. It is not used to deliver the Set ID for document groups.

TXA-2 Dokumenttyp

Dieses Feld typisiert das übermittelte Dokument. Zur genauen Kennzeichnung sollen LOINC-Codes (Document Types) verwendet werden.

This field typed the transmitted document. For accurate identification to LOINC codes (Document Types) are used.

Anmerkung: Im VHitG-Arztbrief wird ein Teil dieser Codes bereits verwendet!

Tabelle 0270: Dokumenttyp

Der Dokumenttyp kann über LOINC-Codes beschrieben werden. Beispielsweise kommen LOINC-Codes in Frage, die als Eigenschaft in der Class-ID folgende Werte haben:

TXA-3 Art der Dokumentenerstellung

Dieses Feld gibt an, in welchem Format das Dokument hinterlegt worden ist, d.h. mit welcher Anwendung/Programm es angezeigt werden kann. Hier sind die Werte aus der Tabelle 0191 zugelassen.

TXA-4 Zeitpunkt der dokumentierten Maßnahme

Dieses Feld gibt an, wann die in dem Dokument angegebene Maßnahme stattgefunden hat.

TXA-5 Verantwortlicher der Maßnahme

In diesem Feld wird der für die in TXA-4 identifizierte Maßnahme verantwortlich ist. Muss vorhanden sein, wenn TXA-4 vorhanden.

TXA-6 Zeitpunkt der Dokumentation

Dieses Feld gibt an, wann das Dokument inhaltlich erfasst worden ist. Das kann dann auch der Zeitpunkt des Diktates sein.

TXA-7 Zeitpunkt der Niederschrift

Dieses Feld gibt an, wann das Dokument textuell erfasst worden ist. Es muss vorhanden sein, wenn TXA-17 einen Wert besitzt, der nicht ‚dictated' ist, d.h. niedergeschrieben worden ist.

TXA-8 Zeitpunkte von Änderungen

Wenn das Dokument verändert worden ist, dann werden hier die Änderungszeitpunkte angegeben. Die Zeitpunkte sollten umgekehrt chronologisch angegeben werden, so dass das jüngste Datum als erstes erscheint. Dadurch können Systeme, die nur das jüngste Datum verwalten, dies ebenfalls verarbeiten.

TXA-9 Autor des Dokuments

TXA-10 Gegenzeichnungsberechtigte Person

In diesem Feld können die Personen angegeben werden, die das Dokument freigeben dürfen/sollen.

TXA-11 Schreibkraft

In diesem Feld wird übermittelt, wer das Dokument elektronisch erfasst hat. Dieses Feld muss bei allen transkribierten Dokumenten gefüllt sein.

TXA-12 Eindeutige Dokumentennummer

In diesem Feld wird eine eindeutige Dokumentnummer angegeben. Dazugehört auch eine Identifikation des ausgebenden Systems. An dieser Stelle wird empfohlen, OIDs (Object Identifier) zu verwenden. In diesem Fall wird als Dokumentennummer die sog. Extension übermittelt. Die Root-OID gibt dann an, welches System diese Nummer vergeben hat. Beide Informationen zusammen sind dann weltweit eindeutig.

Eine Dokumentennummer in diesem Feld ist eindeutig, wenn die in OBX-5 enthaltene Information nicht vom Typ „multipart/related" ist und somit exakt ein Dokument repräsentiert.

Für den Fall des Zusammenfassens mehrerer Dokumente/Dateien ist derzeit nicht geklärt, auf welches Dokument sich die Dokumentennummer in TXA-12 dann bezieht und wie ein evtl. übermitteltes Hauptdokument gekennzeichnet wird.

HL7-D strebt derzeit eine allgemeine Klärung an. Vorbehaltlich des endgültigen Ergebnisses dieser Klärung besteht somit bei Nutzung von „multipart/related" im Zusammenspiel mit der Dokumentennummer in TXA-12 die Gefahr einer nicht profil-konformen Implementierung.

Sobald die Ergebnisse vorliegen wird es ein Update dieses Profils mit entsprechenden Hinweisen geben.

TXA-13 Nummer des Bezugsdokuments

Dieses Feld gibt die Identifikation des Bezugsdokumentes an. Das ist dann die ID des Dokumentes, das ersetzt oder zu dem der Anhang erstellt wird. Es muss vorhanden sein in T05/T06 und T09/T10-Nachrichten.

TXA-14 Auftragsnummer des Auftraggebers

In diesem Feld kann der Bezug zu einem Auftrag über die Auftragsnummer des Auftraggebers („Order Placer Number") hergestellt werden. Additiv dazu stehen auch die Segmente ORC und OBR zur Verfügung, die in dieser Nachricht aber nicht verpflichtend sind.

TXA-15 Bearbeitungsnummer der Leistungsstelle

TXA-16 Eindeutiger Dokumentenname

In diesem Feld wird dem Dokument ein eindeutiger Dateiname ohne Pfadangaben zugeordnet.

Empfehlung: Das Übergabeverzeichnis sollte extern über eine Projektabsprache festgelegt werden, so dass nur der Dateiname übermittelt werden muss. Anmerkung: Sollten entgegen dieser Vorgabe doch Pfadangaben benutzt werden, so ist auf die korrekte Verwendung von Escapezeichen für Backslashes zu achten.

TXA-17 Bearbeitungsstatus

In diesem Feld wird der bereits oben erläuterte Bearbeitungsstatus (Fehler! Verweisquelle konnte nicht gefunden werden.) übermittelt.

TXA-21 Grund der Änderung des Dokuments

In diesem Feld wird der Grund für eine Änderung des Dokumentes in textueller Form (Freitext) übermittelt. Der Grund muss angegeben sein, wenn sich der Status des Dokuments geändert hat.

TXA-22 Person und Zeitpunkt der Gegenzeichnung

In diesem Feld wird angegeben, wer dieses Dokument freigegeben hat. Das können auch mehrere Personen sein.

In der 15. Komponente wird dabei der Zeitpunkt der Freigabe angegeben.

Dieser Wert muss vorhanden sein, wenn der Bearbeitungsstatus in TXA-17 „AU" oder „LA" ist. Dann muss auch die 15. Komponente gefüllt sein.
